TRANSITIONAL AND AFTERCARE SERVICES The mission of the aftercare program is to provide support and assistance to the resident, family/caregiver and community while the individual is transitioning from a therapeutic residential setting through re-integration into community living. The objective of the aftercare program shall be to ensure the individual is prepared for a successful re-integration into a community setting. The aftercare program will monitor the resident’s internalization of skills, prosocial behavior and ability to function in a stable community living environment. This will provide for a reduction in recidivism and reduce the likelihood of reoffending which would necessitate further residential placement. To achieve these objectives the aftercare program will provide a network of support services to the resident, family/caregiver and placing agency. The aftercare program will accomplish this through the following components:
1. Facilitate successful transition to school enrollment
2. Provide or offer resource guide to agency and family/caregiver
3. Assist in preparedness for employment
4. Make referrals for medication management
5. Facilitate referrals to psychiatrist
6. Provide and coordinate mental health services
7. Assist in UA enforcement
8. Maintain weekly communication with family/caregivers and placing agency when appropriate
9. Provide weekly monitoring and face to face visits when needed
10. Provide or coordinate family therapy
11. Monitor attendance to referral programs
12. Provide training to family/caregivers on behavioral modification programs
13. Provide outcome and progress reports to Aurora Plains Academy Clinician
14. Coordinate all referrals to out-patient sex offender, drug and alcohol programs within the community
15. Update consumer outcomes for PQI review
16. Distribute resident, caregiver, and placing agency surveys and forward documentation to appropriate member of the PQI department
